AI Appointment Setter for Med Spas, Clinics and Dentists

How med spas, aesthetic clinics, dental and healthcare practices use an AI appointment setter to answer ad leads and DMs instantly, book consultations, recover no-shows, and stay on the right side of privacy rules.

For med spas, aesthetic clinics and dental practices, an AI appointment setter replies to ad leads, DMs, texts and website chats within seconds, answers questions about treatments, pricing and availability from your own information, books consultations, and follows up with no-shows and lapsed patients. It must also know its limits: no medical advice, and careful handling of health information.

Clinics are some of the clearest cases I've seen for AI setters: expensive ad leads, front desks too busy to text back, and a lot of revenue lost to no-shows. Here's how I'd set one up, using ReplySetter as the example.

The clinic lead problem

  • Ad leads go cold. Someone asks about lip filler on Instagram at 9pm. The front desk sees it at 10am the next day.
  • The front desk is busy with patients in the clinic. Phones, check-ins and payments come first.
  • The same questions repeat: price, downtime, how many sessions, do you take my insurance, are you open Saturday.
  • No-shows cost real money, because the chair or room sits empty.
  • Lapsed patients due for a check-up or top-up never get contacted.

What an AI setter does for a clinic

  1. Answers instantly on Instagram, Facebook, WhatsApp, SMS, website chat and email.
  2. Answers treatment questions from your approved information: what it is, typical price ranges, downtime, how many sessions.
  3. Asks qualifying questions: which treatment, any previous treatments, preferred days, new or existing patient.
  4. Books a consultation on the right calendar or practitioner.
  5. Confirms and reminds, and recovers no-shows with new times.
  6. Reactivates lapsed patients who are due back.
  7. Hands off anything medical or sensitive to the team.

The limits you must set

This is the most important part for healthcare. In the instructions and knowledge base, make these rules explicit:

  • No medical advice or diagnosis. "That's a great question for the practitioner at your consultation."
  • No guarantees of results.
  • Contraindications and medical history go to a clinician, not the AI.
  • Emergencies: tell the person to contact emergency services or the clinic by phone immediately, and hand off.
  • Minimal health information in chat. Collect what's needed to book, not a medical history.

I wrote more on guard-rails in how to stop an AI agent from hallucinating.

Privacy and compliance

Not legal advice, but you should know:

  • In the US, HIPAA applies to covered entities and protected health information. Check whether the tools in your stack (CRM, messaging, AI provider) offer what HIPAA requires, such as a business associate agreement, before handling PHI with them.
  • In the UK and EU, GDPR treats health data as special category data with stricter rules.
  • Marketing texts need consent (TCPA in the US).

A practical approach many clinics take: use the AI for booking and general questions, keep clinical details out of chat, and handle medical intake through their practice management system.

Sample flow for a med spa

Step 1, Greet. "Hi Emma! Thanks for reaching out about lip filler. Have you had it before, or would this be your first time?"

Step 2, Answer and qualify. Answer their question from the knowledge base. Objectives: treatment of interest, first time or returning, preferred days.

Step 3, Book a consultation. "We have consultations Thursday at 4pm or Saturday at 11am with Dr. Lee, which suits?"

Step 4, Confirm. Time, location, parking, what to expect, how to reschedule.

Scenarios:

  • Medical question → defer to the consultation, offer to book.
  • Price question → give the range from the price list, explain the consultation confirms it.
  • Unhappy existing patient → hand off immediately.

Dental practices

Dental adds a few specifics:

  • Insurance questions: load accepted plans into the knowledge base, and have the AI say "we'll confirm your coverage before the visit" rather than guessing.
  • New patient vs existing: route to the right appointment type.
  • Emergencies: toothache and swelling messages should be flagged to the team right away.
  • Recall campaigns: patients due for a check-up or cleaning get a friendly message and a slot. See how to reactivate old leads.

How I'd set it up

With ReplySetter:

  1. Connect channels: Instagram, Facebook, WhatsApp and SMS through GoHighLevel, plus a website chat widget.
  2. Load the knowledge base with treatments, price ranges, downtime, practitioners, hours, location and policies.
  3. Write explicit "never" rules: no medical advice, no guarantees, no diagnosis.
  4. Connect calendars per practitioner or treatment type.
  5. Set no-show recovery so missed consultations get new times right away.
  6. Test with real questions, including tricky medical ones, in the sandbox.
  7. Launch in suggest mode so staff approve replies for the first week.

Frequently asked questions

Can an AI book patients into my clinic's calendar?

Yes, if your calendar is one it can connect to, such as GoHighLevel, Google Calendar, Microsoft 365, Calendly or Cal.com. Practice management systems with an API can be connected as a custom tool.

Is it safe to use AI for patient messages?

For booking and general questions, with clear limits and minimal health information, many clinics do. Check your privacy obligations (HIPAA, GDPR) for your specific setup.

Can it answer pricing questions?

Yes, from the price list you give it. Ranges with "your consultation confirms the exact price" work well.

How does it reduce no-shows?

By confirming, reminding on the patient's preferred channel, making rescheduling easy, and offering new times right after a missed appointment. See how to reduce no-shows.

What if a patient describes a medical emergency?

The AI should tell them to contact emergency services or call the clinic directly, and immediately flag the conversation to your team.
